Physical Therapy appointments are $205, unless discounted in a treatment package (see below). Evaluation appointments do not cost more than a follow up, and services such as dry needling are included in your appointment at no additional fee. Digital Reflection write ups are free for the first 4 visits, and $35 per Reflection after that. You will always be sent the Reflection as written up during your appointment.

We know that recovery takes time, and that everybody progresses at different rates and with different goals. That's why we offer three treatment packages designed to meet your personal wellness goals, follow best clinical practices and standards, and reduce the cost per appointment. Packages are valid for 6 months from the date of purchase, and can be shared among family members. We also offer the option to purchase packages using a payment plan (see below).

Manage Your Pain: Decrease your pain perception, facilitate tissue recovery, and identify harmful movement habits with our 5-treatment package: $975 (5% off per appointment)

Empower Your Body: Develop strength and mobility, form new physiological habits, increase your bodily awareness, and experience newfound confidence in your physical abilities with our 10-treatment package: $1,750 (15% off per appointment)

Progress Your Performance: Improve agility and endurance, reach new levels of physical activity, and experience lasting physiological change with our 15-treatment package: $2,475 (20% off per appointment)

We believe your recovery is an investment for the future. To help make physical therapy more affordable, our treatment packages can now be purchased with a monthly payment plan.

10-treatment package: 2 monthly payments of $875

15-treatment package: 3 monthly payments of $825

Foundation Physical Therapy is a cash-based clinic and is out-of-network (OON) with most health insurance plans and providers. (You may, however, pay for your appointments with an HSA or FSA account.) Most insurance plans will reimburse a percentage of your costs for OON treatments. In other words, your treatments are worth filing! Your appointment cost will help meet your OON deductible, making future appointments as well as other OON claims reimbursable.

To assist you, we have partnered with Reimbursify, a company that specializes in getting reimbursement for out-of-network healthcare. You can verify your out-of-network benefits with the “Use Your Insurance” button or by clicking here. When you fill out your patient intake form, let us know you would like to file for reimbursement. We will make a copy of your insurance ID card at your first appointment and begin submitting your treatments directly to your insurance on your behalf. It’s that easy!

Applying for reimbursement does not guarantee your insurance will accept and reimburse your claim. In the event your claim is denied, we can provide additional information to aid in your claim appeal. Unfortunately, we cannot issue refunds for services already provided. Please note that having a referral or prescription for physical therapy increases the likelihood that your claim will be accepted.

You do not need a doctor’s prescription or referral for an appointment. However, under Georgia law regarding “Direct Access,” there are some limitations. Appointments are limited to 21 days or 8 visits (whichever comes first). To continue care, you will need a referral from your physician, physician’s assistant, or dentist. While this limitation doesn’t apply “in the case of services provided for health promotion, wellness, fitness, or maintenance purposes,” please note that these services may not be reimbursed by your insurance provider. If you plan on filing for reimbursement, or may need care beyond 21 days/8 visits, please reach out to your primary care provider for a referral and have them fax our office at 678-550-9303.
